The Program Director of Case Management will be responsible for the supervision of case managers and discharge planners to promote effective utilization of healthcare resources.
The Case Management Program will provide a collaborative practice model that includes patients, nurses, social workers, physicians, other practitioners, caregivers and the community to facilitate care along a continuum through effective resource coordination to achieve optimal health, access to care and appropriate utilization of internal and external resources balanced with the patient's right to self determination, respect and consideration for his/her wellbeing, required medical services and available family and Community support.
The core functions of the Case Management Program include the following:
- Advocacy & Education
- Clinical Care Coordination/Facilitation
- Continuity/Transition Management
- Performance & Outcomes Management
- Utilization/Financial Management
- Psychosocial Management
- Research & Practice Development
Requirements:
- Must hold a Master's Degree in Nursing
- Must have a minimum of seven (7) years experience as a registered nurse, including four (4) years current, full-time senior/management experience in case management, working in a large North American acute-care, tertiary hospital.
- Experience setting up a case management program is strongly preferred.
- Must hold a current RN license from Canada or the USA.
