Serves as a pharmaceutical caregiver by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.
Positions available in the following departments: Critical Care, PICU, Infectious Diseases, Transplant, Cardiology
Requirements:
- BScPharm, Masters Degree or Doctor of Pharmacy required
- Current licensure as a Clinical Pharmacist in Canada or the United States, and
- For those with a Bachelor's degree in Pharmacy: Six (6) years of hospital pharmacy practice including three (3) years in clinical capacity in a large North American acute-care hospital.
- For those with a Master's in Pharmacy or PharmD: Two (2) years of hospital pharmacy experience in a large North American acute-care hospital.
